In hereby represent that I have read the information clause on processing my personal data in connection with my participation in the Polish-African Economic Forum (hereinafer:”Forum”) and hereby grant a voluntary consent for the processing by PAIH S.A. with its registered office at ul. Krucza 50 (00-025 Warsaw) of my personal data that I have provided in the registration form in order to participate in the Forum. I know that I can withdraw my consent at any moment. The withdrawal of consent does not affect the lawfulness of processing based on the consent before its withdrawal.

Consent to transfer personal data to the Department of Tourism and Commerce Marketing in Dubai*

In hereby grant my voluntary consent for PAIH S.A. with its registered office at ul. Krucza 50 (00-025 Warsaw) to transfer my personal data to the Department of Tourism and Commerce Marketing in Dubai, seated in a third state, i.e. a country outside the European Economic Area in connection with my participation in the Polish-African Economic Forum. I am aware of any risk that such an event may entail due to a lack of decision to confirm an appropriate security level or absence of proper protective measures, in particular a risk resulting from an impossibility to ensure personal data protection in a standard set forth in the domestic and EU personal data protections laws. I know that I can withdraw my consent at any moment. The withdrawal of consent does not affect the lawfulness of processing based on the consent before its withdrawal.

Consent for the processing data in a form of an image*

In hereby represent that I have read the information clause on processing my personal data in a form of an image and hereby grant a voluntary consent for the processing, in particular by recording, using and dissemination of my image in a form of a photograph or recording made during my participation in the Polish-African Economic Forum by Polska Agencja Inwestycji i Handlu S.A. (the Polish Investment and Trade Agency) with its registered office in Warsaw at ul. Krucza 50 (00-025 Warsaw). I know that I can withdraw my consent at any moment.
